Erken Dönem Tip 2 Diyabetes Mellitus Hastalarında Postprandiyal Rahatsızlık Sendromu ‘nun Yaşam Kalitesi Üzerine Etkilerinin Değerlendirilmesi

Abstract

Amaç: Erken dönemde tanı konulmuş ve komplikasyon gelişmemiş tip 2 diyabetes mellitus hastalarında ve sağlıklı kontrol grubunda yaşam kalitesi skorlarını karşılaştırmak ve yaşam kalitesine etki eden faktörlerin belirlenmesidir.. 

Materyal-Metot:Bu vaka kontrol çalışması İç Hastalıkları polikliniklerine başvuran 151 hasta ile yapılmıştır. Araştırmaya erken dönemde tanı konulmuş ve komplikasyon gelişmemiş olan tip 2 diyabetes mellitus hastası ve herhangi bir sistemik hastalığı ve ilaç kullanımı olmayan benzer yaş ve cinsiyette olan 92 hasta randomizasyonla kontrol grubu olarak dahil edildi. Hastaların cinsiyet, yaş, boy, kilo, sigara, alkol kullanımları, postprandiyal sendrom varlığı sorgulandı. Ayrıca hastaların yaşam kaliteleri Kısa Form Sağlık ölçeği testi aracılığıyla sorgulandı. Analizler SPSS programı aracılığıyla test edildi. 

Bulgular:Hasta grubu ile kontrol grubu cinsiyet (p=0.430), yaş (p=0.857), sigara (p=0.807) ve alkol kullanımı (p=0.745) açısından benzerdi. Hasta grubunun body mass index  ortalamaları kontrol grubuna göre daha yüksekti (p=0.001) ve hasta grubunda postprandiyal sendromu sıklığı (p=0.001) daha fazlaydı. Hasta grubunun Psikolojik komponent skoru  puan ortalamaları tek değişkenli analizlerde daha düşükken (p=0.006), çok değişkenli analizlerde hasta grubu ile kontrol grubu arasında fark yoktu. Psikolojik komponent skorlarının çok değişkenli analizlerde vücut kitle indeksi  ıyüksek olanlarda ve postprandiyal sendromu olanlarda daha düşük olduğu görüldü. 

Sonuç: Çalışmamızda erken dönemde tanı alan ve komplikasyonu bulunmayan tip 2 diyabetes mellitus hastalarında yaşam kalitesinin diyabetle ilişkili olarak düşmediği tespit edilmiştir. Postprandiyal sendromun ve yüksek vücut kitle indeksinin yaşam kalitesi üzerine diyabet varlığından daha fazla etkileri olduğu gözlemlendi.
