BİLATERAL HİDRONEFROZUN NADİR SEBEBİ: MESANE HERNİASYONU

Abstract


Mesanenin inguinoskrotal herniasyonu seyrek görülen, çoğunlukla asemptomatik patolojidir.Mesane herniasyonu arasıra mesane nekrozu, obstrüktif üropati gibi ciddi komplikasyonlara neden olabilir. Mesane tabanın yer değiştirmesi, üreterin inguinoskrotal herni kesesine katılması, üreterin obstrüksiyonu hidronefroz gelişimine sebep olabilir. Mesane hernisi cerrahi müdahele öncesi belirlenemezse, inguinal herni tamir operasyonları sırasında mesane yaralanması gelişebilir. Retrograd sistografi, intravenöz piyelografi, ultrasonografi gibi radyolojik yöntemler tanıda kullanılabilir. Bilgisayarlı tomografide (BT) mesane tabanının anteroinferiora açılanma göstermesi; büyük boyutlu hernilerde mesanenin inguinal kanal boyunca, skrotuma ilerlediğinin belirlenmesi tanı için önemli bulgulardır. Multiplanar rekonstrüksiyon ile elde olunan koronal ve sagital BT görüntüleri tanıda fayda sağlar.

RARE CAUSE OF BILATERAL HYDRONEPHROSIS: URINARY BLADDER HERNIATION

Abstract


Inguinoscrotal herniation of urinary bladder is an uncommon and usually asyptomatic pathology. Urinary bladder herniation sometimes may cause serious complications like urinary bladder necrosis, obstructive uropathy. Displacement of urinary bladder basis, participation of ureter to inguinoscrotal herniation and ureter obstruction can develop hydronephrosis. If it is not detected before operation, urinary bladder injuries may occur during herniorrhaphies. Radiologic methods such as retrograde cystography, intravenous pyelography and ultrasonography can be used for diagnosis. At computed tomography (CT) anteroinferior angulation of urinary bladder basis and migration of bladder to scrotum along the inguinal canal in large size herniations are significant findings. Sagittal and coronal CT images obtained by multiplanar reconstruction are benefical for diagnosis
