The role of fibromyalgia in rheumatic diseases

Abstract

Fibromyalgia syndrome (FMS) is a clinical disorder characterized by chronic diffuse musculoskeletal pain, numerous discrete tender points and multiple somatic and cognitive symptoms, without any specific laboratory and imaging findings. Prevalence of FMS is higher in chronic inflammatory arthritis, compared to the general population. Comorbid FMS may lead to increased disease activity scores, leading to an inaccurate impression of disease severity. Thus, it is important to recognize comorbid FMS for the optimal management of rheumatic diseases. Disease activity indices should be cautiously interpreted, and objective clinical measurements should be considered in patients with comorbid FMS. In this review, the prevalence and pathophysiology of concomitant FMS in different rheumatic diseases and its impact on underlying inflammatory condition will be highlighted.

Romatolojik hastalıklarda fibromiyaljinin payı

Abstract

Fibromiyalji sendromu (FMS), kronik yaygın kas-iskelet ağrısı, yaygın hassas noktalar, multipl somatik ve bilişsel problemlerle karakterize, spesifik laboratuvar ve görüntüleme bulgusu olmayan klinik bir sendromdur. Kronik romatolojik hastalıklarda FMS prevalansının genel popülasyona göre daha yüksek olduğu gösterilmiştir. Komorbid FMS hastalık aktivite skorlarının yükselmesine, dolayısıyla romatolojik hastalığın şiddetinin yanlış değerlendirilmesine ve gereksiz tedavilerin verilmesine neden olabilir. Bu nedenle eşlik eden FMS’nun tanınması, romatolojik hastalığın optimal tedavisi için önem taşır. Komorbid FMS olan olgularda hastalık aktivite göstergeleri dikkatle yorumlanmalı, objektif klinik ölçütler göz önünde bulundurulmalıdır. Bu derlemede, değişik romatolojik hastalıklarda komorbid FMS prevalansı, patofizyolojisi ve altta yatan inflamatuvar hastalık üzerindeki etkileri gözden geçirilecektir.
