Investigation of the relationship between the amounts of tissue removed in breast reduction operations and the change in symptoms

Abstract

Aim: Breast reduction operations are frequently performed operations with satisfactory both aesthetic and functional results. In this article, we presented our study in which we investigated the effect of the amount of tissue resected during the breast reduction operation on the symptoms.
Materials and Methods: 57 patients were included in the study. The groups were divided into 3 groups according to the weight of the tissue resected from both breasts. Group 1: 1000 grams of tissue and less, group 2: 1000-2000 grams of tissue, group 3: The group in which 2000 grams and more tissue is resected. Patients' ages, body mass indexes (BMI), total amount of tissue removed from the bilateral breasts were noted. Before and after the operation, neck/back/shoulder pain, redness and itching in the breast area, bra marks and symptoms of limitation of movement in daily life of the patients were evaluated and compared. The correlation between BMI and the amount of tissue removed was evaluated.
Results: While there was a significant difference between the groups in terms of the amount of tissue removed and BMI, there was no significant difference in age. Back/shoulder pain in group 2-3, neck pain, redness-itch in group 3, bra scar and limitation of movement changed significantly in all groups. The preoperative shoulder pain, bra mark and limitation of motion score in group 3 and the postoperative bra mark score in group 3 were found to be significantly higher than other groups. When the correlation between the amount of tissue removed and BMI was evaluated, a significant positive correlation was found in group 3 in terms of two values.
Conclusion: Symptomatic improvement after breast reduction surgery is not significantly different among women with different breast sizes.

Abstract

Amaç: Meme küçültme operasyonları sıklıkla yapılan hem estetik hem de fonksiyonel sonuçları yüz güldürücü olan operasyonlardır. Çıkartılan doku miktarı ile semptomların iyileşmesi ile ilgili literatürde kısıtlı sayıda çalışma mevcuttur. Biz bu yazımızda meme küçültme operasyonu sırasında rezeke edilen doku miktarının semptomlara etkisini araştırdığımız çalışmamızı sunduk.
Gereç ve Yöntem: Çalışmamız Ocak 2018-Nisan 2021 tarihleri arasında yapılmış retrospektif bir çalışmadır. Çalışmaya 57 hasta dahil edildi. Gruplar her iki memeden çıkarılan dokunun ağırlığına göre üç gruba ayrıldı. Grup 1: 1000 gram ve daha az doku, grup 2: 1000-2000 gram arası doku, grup 3: 2000 gram ve daha fazla dokunun çıkarıldığı grup. Hastaların yaşları, beden kitle indeksleri (BKİ), bilateral memeden çıkartılan toplam doku miktarları not edildi. Operasyon öncesi ve sonrası boyun ağrısı, sırt ağrısı, omuz ağrısı, meme bölgesindeki kızarıklık ve kaşıntı, sutyen izi ve hastaların günlük hayattaki hareket kısıtlılığı semptomları değerlendirilmiş ve karşılaştırılmıştır. BKİ ile çıkartılan doku miktarı arasındaki korelasyon değerlendirildi.
Bulgular: Çıkartılan doku miktarı ve BKİ açısından gruplar arasında anlamlı bir fark varken, yaş arasından anlamlı bir fark saptanmadı. Operasyon öncesi ve sonrası değerlendirildiğinde, sırt ağrısı, omuz ağrısı grup 2 ve grup 3’te, boyun ağrısı, kızarıklık-kaşıntı grup 3’te, sutyen izi ve hareket kısıtlılığı ise üç grupta da anlamlı olarak değişmiştir. Grupların preoperatif ve postoperatif değerleri kendi arasında değerlendirildiğinde; Grup 3’te preoperatif omuz ağrısı, sutyen izi ve hareket kısıtlılığı skoru ve yine grup 3’te postoperatif sutyen izi skoru diğer gruplardan anlamlı olarak büyük saptanmıştır. Çıkartılan doku miktarı ve BKİ arasındaki korelasyon değerlendirildiğinde grup 3’te iki değer açısından anlamlı bir pozitif korelasyon saptandı.
Sonuç: Meme küçültme operasyonu sonrası oluşan semptomatik iyileşme, farklı meme boyutlarına sahip kadınlar arasında önemli ölçüde farklı değildir.
