Mamografide izlenen kuşkulu lezyonların iğne ile işaretlenerek ultrasonografi kılavuzluğunda yapılan kalın iğne biyopsi sonuçları

Öz

Amaç Mamografi ile meme kanseri taramasının artışı memede saptanan nonpalpabl lezyonların oranını arttırmaktadır. Sadece mamografi ile görülebilen kuşkulu lezyonların tanısında stereotaktik vakum aspirasyon biyopsi (VAB) tercih edilen ilk yöntemdir. Ancak vakum biyopsi ünitesi ve vakum iğnesinin yüksek maliyetli olması nedeniyle bu yönteme erişim sınırlıdır. Bu çalışmada VAB’ ye alternatif olarak mamografide izlenen kuşkulu lezyonun ince iğne ile işaretlenmesi ardından, bu iğne kılavuzluğunda US eşliğinde yapılan kalın iğne biyopsi sonuçlarının değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem Ocak 2021-Nisan 2022 tarihleri arasında, sadece mamografide izlenebilen ve kuşkulu kategoride olan lezyonlara, mamografide ince iğne ile işaretleme ardından, US rehberliğinde iğnenin bulunduğu alana kalın iğne biyopsi yapılan hastalar retrospektif olarak taranmıştır. Hastaların mamografi bulguları, lezyonun BI-RADS kategorisi, biyopsi örnekleme sayısı, spesmen mamografisi, biyopsi patoloji sonuçları, varsa cerrahi eksizyon patoloji sonuçları değerlendirilmiştir. Bulgular Biyopsi yapılan toplam 43 hastanın 39’unda sadece kuşkulu mikrokalsifikasyon, 3’ ünde sadece asimetri ve distorsiyon, 1 ‘inde mikrokalsifikasyon ve eşlik eden asimetri izlenmekteydi. Kalın iğne biyopsi histopatoloji sonuçlarında %58 benign, %9 atipili benign ve %33 malign tanı saptandı. 15 hastada cerrahi eksizyon ile lezyon çıkarıldı, radyoloji patoloji uyumu olan diğer hastalar takibe alındı. 43 hastanın %39.5’i malign, %60.5’i benign gruptaydı. Mamografide yerleştirilen ince iğnenin kılavuzluğunda yapılan, kalın iğne biyopsi işleminin tanısal doğruluğuna bakıldığında; sensitivite %76.5, spesifite %100 olarak saptandı. Sonuç Mamografide saptanan kuşkulu mikrokalsifikasyonlar erken evre meme kanserinin tanısında önemli belirteçlerdir. Kuşkulu lezyonların tanısında mamografide yerleştirilen ince iğnenin rehberliğinde yapılan trucut biyopsi yüksek tanısal doğruluğa sahip, açık cerrahi biyopsi ve VAB’ ye alternatif olarak kullanılabilecek minimal invaziv bir yöntemdir.

Ultrasound guided core needle biopsy results after marking suspicious lesions on mammography with a fine needle

Abstract

Aim The increase in breast cancer screening with mammography increases the rate of nonpalpable lesions detected in the breast. Stereotactic vacuum aspiration biopsy (VAB) is the first method of choice for the diagnosis of suspicious lesions that can only be seen with mammography. However, access to this method is limited due to the high cost of the vacuum biopsy unit and vacuum needle. In this study, as an alternative to VAB, it was aimed to evaluate the results of core needle biopsy performed under US guidance, after the suspicious lesion observed on mammography was marked with a fine needle. Material and Method Between January 2021 and April 2022, patients who underwent a US-guided core needle biopsy to the area where the needle was located, followed by fine-needle marking on mammography, were retrospectively screened. Mammography findings of the patients, BI-RADS category of the lesion, number of biopsy samples, specimen mammography, biopsy pathology results, and surgical excision pathology results, if any, were evaluated. Results Of 43 patients who underwent biopsy, only suspicious microcalcification was observed in 39, only asymmetry and distortion in 3, microcalcification and accompanying asymmetry in 1 patient. In the core needle biopsy histopathology results, 58% were benign, 9% were benign with atypia, and 33% were malignant. The lesion was removed by surgical excision in 15 patients, and other patients with radiology-pathology compatibility were followed up. Of 43 patients, 39.5% were in the malignant group and 60.5% in the benign group. Considering the diagnostic accuracy of the core needle biopsy performed under the guidance of the fine needle placed in mammography; sensitivity was 76.5% and specificity was 100%. Conclusion Suspicious microcalcifications detected on mammography are important markers in the diagnosis of early-stage breast cancer. Tru-cut biopsy performed under the guidance of a fine needle inserted in mammography in the diagnosis of suspicious lesions is a minimally invasive method with high diagnostic accuracy that can be used as an alternative to open surgical biopsy and VAB.
