Comparison of the videothoracoscopic and open lung biopsies in the diagnosis of interstitial lung diseases

Abstract

Background: Lung biopsy is required, when interstisyel lung disease (ILD) can't be diagnosed with radiological research, transbronchial biopsy and bronchoalveoler lavage. In this instance, it comes into question that which biopsy method - videothoracoscopic (VT) or open lung biopsy (OLB) - is suitable for the patient. In this study our experience in lung biopsy via videothoracoscopic and open lung biopsy for ILD patients is presented. Material and method: Fiftyfour patients who underwent lung biopsy via VT and OLB methods for a certain diagnose of ILD in our clinic in the past ten years were reviewed retrospectively. The data regarding age, sex, radiological appearance, number of biopsies, operative time, duration of chest tube, postoperative hospital stay, complications, diagnostic yield and costs were obtained. Results: The mean age and the range of the 35 (%65) female and 19 (%35) male patients in VT and OLB groups were 50.27±13.77 (24-78) and 44.00±13.69 (25-78) respectively. Thirtythree (%61) VT biopsy and 15 OLB was performed while in 6 (%11) VT biopsy patients conversion to OLB was required. Most of the certain diagnosis (%55.55) were nonspesific interstitial fibrosis, bronchiolitis obliterans organizing pneumonia and granulomatous lung disease. Operative time, duration of chest tube and postoperative hospital stay were significantly shorter in VT group (p values 0.004, 0.036, 0.034 respectively). In the cost analyse it was determined that VT was significantly expensive than OLB (p=0.0001). Discussion: In our opinion, in spite of high costs, VT should be the first choice for the diagnosis of ILD, because of it's shorter operative time and postoperative hospital stay and less scar formation. OLB should be prefered for the patients who aren't candidates for VT biopsy.
