Colonic volvulus: continuing surgical problem and treatment options

Abstract

Aim: Colonic volvulus is a cause of acute abdomen which needs prompt diagnosis and treatment. There are a number of different surgical and nonsurgical treatment alternatives. Because of this reason we investigated results of patients operated for colonic volvulus and presented in this study. Material and Methods: Twenty eight patients operated for colonic volvulus at 1st Surgery Clinics at Izmir Bozyaka Education and Research Hospital were included and their records were retrospectively investigated. Results: Twenty eight colonic volvulus at 27 patients were operated at a 10 year period. Eleven(40,7 %) of the 27 cases were female, whereas 16(59,3) were male. The average age of the patients were 69(28-95). Twenty (70%) of the patients were preoperatively diagnosed; 10 of this 20 patients were treated with emergency laparotomy whereas the other 10 patients were treated with endoscopic decompression. Sixteen (56%) patients were treated with resection and colostomy, 8(28%) patients were treated with resection and anastomosis, 2 (7%) patients were treated with detortion, 1 patient was treated with detortion and sigmoidopexy, 1 patient was treated with resection and ileostomy. Mortality was seen at 2 cases (7%). Conclusion: Endoscopic decompression should be the first choice for suitable patients. Surgical treatment is inevitable if ischemia, gangren or perforation is present or if endoscopic decompression fails.
