Colonic volvulus: continuing surgical problem and treatment options

Aim: Colonic volvulus is a cause of acute abdomen which needs prompt diagnosis and treatment. There are a number of different surgical and nonsurgical treatment alternatives. Because of this reason we investigated results of patients operated for colonic volvulus and presented in this study. Material and Methods: Twenty eight patients operated for colonic volvulus at 1st Surgery Clinics at Izmir Bozyaka Education and Research Hospital were included and their records were retrospectively investigated. Results: Twenty eight colonic volvulus at 27 patients were operated at a 10 year period. Eleven(40,7 %) of the 27 cases were female, whereas 16(59,3) were male. The average age of the patients were 69(28-95). Twenty (70%) of the patients were preoperatively diagnosed; 10 of this 20 patients were treated with emergency laparotomy whereas the other 10 patients were treated with endoscopic decompression. Sixteen (56%) patients were treated with resection and colostomy, 8(28%) patients were treated with resection and anastomosis, 2 (7%) patients were treated with detortion, 1 patient was treated with detortion and sigmoidopexy, 1 patient was treated with resection and ileostomy. Mortality was seen at 2 cases (7%). Conclusion: Endoscopic decompression should be the first choice for suitable patients. Surgical treatment is inevitable if ischemia, gangren or perforation is present or if endoscopic decompression fails.

Amaç: Kolon volvulusları kısa sürede tanı konulup uygun tedavi gerektiren bir akut batın sebebidir. Sağaltımında nonoperatif girişimlerden operatif tedaviye kadar birçok alternatif seçenekler uygulanabilmektedir. Bu sebeple kolon volvulusu nedeniyle opere edilen olguların sonuçları değerlendirilmiş ve bu çalışmada sunulmuştur. Yöntem ve Gereç: İzmir Bozyaka Eğitim ve Araştırma Hastanesi 1. Genel Cerrahi Kliniği'nde opere edilen 28 kolon volvulusu olgusu çalışmaya dahil edilmiş ve kayıtları retrospektif olarak incelenmiştir Bulgular: On yıllık dönemde 27 olguda 28 kolon volvulusu opere edilmiştir. Olguların 11 (%40,7)'i kadın, 16 (%59,3)'sı erkek olup ortalama yaş 69 (28-95)'dur. Preop olguların 20 (%70)'sine tanı konulmuş olup 10'una acil laparotomi, 10'una endoskopik dekompresyon uygulanmıştır. Onaltı (%56) olguya rezeksiyon + kolostomi, 8 (%28) olguya rezeksiyon + anastomoz, 2 (%7) olguya detorsiyon, 1 (%3,5) detorsiyon + sigmoidopeksi, 1 (%3,5) olguya rezeksiyon + ileostomi uygulanmıştır. Mortalite 2 (%7) olguda görülmüştür. Sonuç: Genel durumu uygun hastalarda primer tedavi endoskopik dekompresyon olmalıdır. Cerrahi tedavi iskemi, gangren ve perforasyon varlığında yada başarısız endoskopik dekompresyon sonrası kaçınılmazdır.
