Result of onlay mesh closure for patients having giant midline incisional hernias

Abstract

Aim: We aimed to assess the results of onlay propylene mesh reconstruction without closing up the fascia of midline incisional hernias larger than 10cm in this study. Materials and Methods: Patients who underwent surgery for incisional hernia fascial defects larger than 10 cm of the midline abdominal fascia closure, and on which the open onlay mesh technique had been used, were analyzed retrospectively. Thirty-one patients who could be reached by telephone, were included in the study and the control was determined. Factors such as gender, age, additional diseases, reasons for the first surgery, defect size, body mass index, primary-recurrence of incisional hernia, duration of hospital stay, postoperative complications and recurrence were investigated. Results: Thirteen patients (41.9%) who relapsed at least once had incisional hernias. Eighteen patients (58.1%) had an additional disease. Fascial defects were found in patients with an average diameter of 17.8 cm (10-50 cm), respectively. Additionally, five patients in the postoperative period (16.19%), seroma, one (3.2%), wound infection, one (3.2%), enterocutaneous fistula were confirmed. Six patients (19.4%) had recurrence and relapse, all within the first two years (mean 12.5 months, 3-24 months). Conclusion: Incisional hernia is a complication of abdominal surgery and is common and difficult. Additional problems, encountered especially for patients with large incisional hernias make the situation more complicated. For those patients, good preoperative preparation and the use of the patch are required. Application of an onlay patch, before the closing of the fascia, to prevent tension is considered as a simple and dependable procedure.
