Incidence of pyramidal lobe on thyroid scintigraphies

Abstract

Introduction: The pyramidal lobe, an embryological remnant of the thyroglossal duct, extends up anatomically from the lobes or isthmus of the tyroid gland. It is important to know the localization of pyramidal lobe in the cases who will undergo to total thyroidectomy operation. Materials and Methods: The Tc-99m pertecnetate thyroid scintigraphies were performed on 805 patients (616 women, 189 men) aged between 19 to 78 years, in order to determine the incidence of pyramidal lobe on thyroid scans. Result and Discussion: The pyramidal lobe was visualized in 20.8% of cases. The frequency of appearance was slightly higher in males than females. Pyramidal lobe was usually located on the left lobe in both sex. Interestingly in two cases (one woman and one man) it was found that two pyramidal lobes extending from the each lobes of thyroid. Investigating thyroid function studies of the patients with a pyramidal lobe on their scans we determined the high incidence (79.1%) of euthyroidism. Only 12.5% and 8.3% of the patients showed hyperthyroidism and hypothyroidism respectively. All of the patients with hyperthyroidism had the findings of diffuse toxic goitre. Pyramidal lobe should be taken into consideration in patients referred for total thyroidectomy operation. Before thyroid operation, Tc-99m pertechnetate scan will be helpful in the localization of pyramidal lobe, because it is a functional imaging modality.

Öz

Giriş: Tiroglossal duktusun embriyonel bir kalıntısı olan piramidal lop, anatomik olarak tiroit loplarından veya istmustan yukarı doğru uzanır. Total tiroidektomi uygulanacak olan olgularda piramidal lobun nerede yerleştiğinin bilinmesi önemlidir. Gereç ve Yöntem: Piramidal lobun tiroit sintigrafilerinde görülme sıklığını saptamak üzere, yaşları 19 ile 78 arasında değişen 805 hastaya (616 kadın, 189 erkek) Tc-99m perteknetat ile tiroit sintigrafisi yapıldı. Sonuç ve Tartışma: Olguların %20.8 inde piramidal lop görüntülendi. Görülme sıklığı erkeklerde kadınlara oranla biraz daha fazlaydı. Piramidal lop her iki cinste de sıklıkla sol lobta yerleşmişti. İlginç bir bulgu olarak, biri kadın diğeri erkek iki olguda tiroidin iki lobundan çıkmış iki adet piramidal lop varlığı dikkati çekti. Sintigrafilerinde piramidal lop saptanan olguların tiroit fonksiyonlarını araştırdığımızda, olguların çoğunun (%79.1) ötiroidik, daha az bir bölümünün ise hipertiroidik (%12.5) ve hipotiroidik (%8.3) olduğunu gördük. Hipertiroidik olguların tümü diffüz toksik guatr bulgularına sahipti.Piramidal lobun özellikle total tiroidektomi operasyonuna alınacak hastalarda gözden kaçırılmaması gerekir. Tiroit ameliyatlarından önce çekilen Tc-99m pertknetat sintigrafisi, fonksiyonları görüntüleyen bir çalışma olduğundan piramidal lobun lokalize edilmesine yardımcı olur.
