HISTOPATHOLOGIC FEATURES İN CHRONIC HEPATITIS C: A COMPARATIVE STUDY WITH CHRONIC HEPATITIS B

Abstract

The aim of this study is to compare the histopathologic features observed in chronic hepatitis C (CHC) and chronic hepatitis B (CHB) in order to search for diagnostic clues. Liver biopsy materials, taken prior to İnterferon treatment from patients with clinico-pathological diagnoses of CHC (n:37) and CHB (n:23), betvveen 1989 and 1995 were included in the study. Histopathologic examination was performed for the detection of steatosis, acidophil bodies, bile duct lesions, activation of sinusoidal inflammatory cells, lymphoid aggregates and/or follicles in portal tracts, ground glass hepatocytes, nuclear polymorphism, sanded nuclei, Mallory body-like material, emperipolesis, pehpolesis, acidophilic change and large celi change (LCC) of hepatocytes. Most common features were; lymphoid aggregates and/or follicles in portal tracts (%86,48), pehpolesis (%70,27), steatosis (%51,35), acidophil bodies (%48,64), nuclear polymorphism (%43,24), bile duct lesions (%37,83), acidophilic change (%35,13), activation of sinusoidal inflammatory cells (%35,13), in CHC, and peripolesis (%91,30), nuclear polymorphism (%86,9), ground glass hepatocytes (%78,26), acidophilic change (%65,21), acidophil bodies (%65,21), sanded nuclei (%52,17) and emperipolesis (%52,17) in CHB. Öz

Bu çalışmada kronik hepatit C (KHC) ve kronik hepatit B (KHB)' de izlenen histopatolojik özellikleri karşılaştırıp tanısal değeri olan ipuçları araştırmak amaçlandı. Kliniko-patolojik olarak KHC (n: 37) ve KHB (n: 23) tanısı konan hastaların interferon tedavisi öncesinde alınan biyopsi materyalleri yağlanma, asidofil cisim, safra kanalı lezyonu, sinüzoidal yangısal hücre aktivasyonu, portal lenfoid agregat ve/veya follikül oluşumu, buzlu cam hücreleri, nükleer polimorfizm, kumlu nukleus, Mallory-benzeri cisim, emperipolezis, peripolezis, hepatositlerde asidofil değişiklik ve büyük hücreli değişiklik (BHD) açısından değerlendirildi. En sık görülen özellikler KHC'de portal lenfoid agregat ve/ veya follikül oluşumu (%86,48), peripolezis (%70,27), yağlanma (%51,35), asidofil cisim (%48,64), nükleer polimorfizm (%43,24), safra kanalı lezyonu (%37,83), asidofil değişiklik (%35,13), sinüzoidal yangısal hücre aktivasyonu (%35,13), KHB'de ise peripolezis (%91,30), nükleer polimorfizm (%86,9), buzlu cam hücreleri (%78,26), asidofil değişiklik (%65,21), asidofil cisim varlığı (%65,21), kumlu nukleuslar (%52,17) ve emperipolezis (%52,17) idi.
