THE USE OF TUNICA VAGINALIS GRAFT IN THE SURGICAL THERAPY OF PEYRONIE'S DISEASE

Abstract

Peyronie's disease is an inflammatory disorder of the tunica albuginea of the penis. Excision of the plaque and grafting is the another treatment of choice for the correction of penil deformity. The aim of this study was to evaluate the outcomes of tunica vaginalis graft repair after Peyronie's plaque excision. From 1996 to 1999, 7 patients with at least 1 year history of Peyronie's disease with penil curvature greater than 45 degrees were evaluated. Their mean age as 51 ± 6.5 (range:37-58) years and the mean duration of disease was 32.3 ± 13.7 (range: 12-64) months. Ali of them had normal sexual potency. Autolog tunica vaginalis graft was used to close the corporotomy defect after plaque excision, via a subcoronal incisional approach. Eleven graft were used in 7 patients with average 1.8 cm2 surtace area. Surgical correction of the curvature was achieved in ali patients. At mean follow-up of 23 ± 7.9 (range: 8-36) months, ali patients were potent vvithout requiring medical therapy. No complaints ofpenile shortening and bulging at the graft site have been noted. One patient reported a 30-degree curvature in operated site after 9 months postoperatively. Overall patient satisfaction with the procedure was 100 %. The use of tunica vaginalis grafts for the coverage of corporotomy defects after Peyronie's plaque excision is a simple, safe method with no additional expense and allows for satisfactory clinical results. These preliminary results need to be confirmed in larger series.
