ANTALYA İL MERKEZİNDEKİ ÖĞRENCİLERDE BOY KISALIĞI PREVALANSI VE BOY PERSENTİL EĞRİLERİ

The purpose of this study is to determine short stature prevalence and height percentiles in a cohort of students in the city center of Antalya. A total of 2465 students (boys 1233, girls 1232) aged between 6 to 17 years, were chosen by using population based stratified cluster sampling method representing all the students in the city center of Antalya, within the same age group. Standing height was measured using portable stadiometer to the nearest 0.1 cm. Short stature was defined as height z score is below -2 standard deviation. The overall short stature prevalence was 2.2% (2.1% in boys and 2.3% in girls, p>0.05). Height percentile curves were constructed by using LMS method. 0.62nd percentile curve has been representing the pathological short stature percentile curve and -2.5 standard deviation. Regarding final heights, it was found that girls were 2.8 cm higher, and boys were 2.9 cm higher than those of values used in the Turkish boys' height percentile chart. It may be beneficial for doctors working in the first step health centers to use 0.62nd percentile curve in the standard height percentile charts to reduce risk for skipping pathological short stature. On the other hand, we think that it is crucial to revise or update the currently used height percentile charts of Turkish children.

Çalışmamız Antalya il merkezindeki öğrencilerde boy kısalığı prevalansını ve boy persentil eğrilerini belirlemek amacıyla yapıldı. Yaşları 6-17 arası değişen toplam 2465 öğrenci (1233 erkek, 1232 kız), Antalya il merkezindeki kendi yaş grubundaki tüm öğrencileri temsil edebilecek şekilde, nüfusa orantılı tabakalı küme örnekleme yöntemi kullanılarak seçildi. Boy ölçümleri taşınabilir Harpenden stadiyometre kullanılarak 0.1 cm hassasiyetle yapıldı. Z skoru -2 SDS'den küçük olan öğrenciler kısa boylu olarak kabul edildi. Toplamda boy kısalığı prevalansı %2.2 olarak saptandı. Bu verilerin cinsiyete göre dağılımları incelendiğinde; boy kısalığı prevalansı erkeklerde %2.1, kızlarda ise %2.3 idi (p>0.05). LMS metodu kullanılarak Antalya il merkezindeki öğrencilerin boy persentil eğrileri oluşturuldu. 0.62.persentil eğrisi -2.5 SD'ye denk gelmekteydi ve patolojik boy kısalığı eğrisini temsil etmekteydi. Bu değerler halen kullanılmakta olan Türk çocukları için boy persentil değerleri ile karşılaştırıldığında; final boyda erkek çocukların ortalama 2.9 cm, kız çocuklarının ise ortalama 2.8 cm daha uzun olduğu saptandı. Standart boy persentil eğrilerinde bulunmayan 0.62. persensentil eğrisinin kullanılması, özellikle birinci basamak sağlık hizmetinde çalışan hekimlerin patolojik boy kısalıklarını gözden kaçırma riskini azaltabilir. Diğer yandan 25 yıl önce hazırlanan ve halen kullanılmakta olan standart boy persentil eğrilerinin güncellenmesi gerektiği düşüncesindeyiz.
