Hepatit C virusu ile infekte hastalarda hepatit A ve hepatit B virus serolojileri

As acute infection with hepatitis A (HAV) or B virus (HBV) in those with underlying chronic hepatitis C virus (HCV) infection can result in high morbidity, HAV and HBV vaccination should be applied to those who are seronegative for these viruses. In this study, we investigated HAV and HBV serologies in the patients with HCV infection. This study was conducted retrospectively. The consecutive patients addmitted to Infectious Diseases Clinic between April 2001 and April 2006 were included in the study. All subjects were evaluated for HAV and HBV serological markers (anti-HAV IgG, HBsAg, anti-HBs and anti-HBcIgG). At the end of the study we found that these serological markers were investigated in 51 subjects. In these patients, anti-HAV IgG, HBsAg, anti-HBs, anti-HBs+anti-HBc IgG positivity rates were found as 68.6%, 1.9%, 13.7% and 29.4%, respectively. Sixteen patients who were seronegative for HAV were vaccinated by HAV vaccine. Twenty eight patients were seronegative for HBV. These were vaccinated by HBV vaccine. In conlusion, 30% and 50% of the patients with HCV were found to be seronegative for HAV and HBV, respectively. HAV and HBV vaccination are recommended for these patients.

Hepatit C virusu (HCV) ile infekte hastalarda, hepatit B (HBV) ya da hepatit A virus (HAV)'larından birinin bulaşı infeksiyonun seyrini olumsuz etkilemektedir. Bu nedenle, hepatit C'li hastalarda, aşı ile korunulabilen bu iki infeksiyona karşı profilaksi uygulanması önerilmektedir. Bu çalışmada, infeksiyon hastalıkları polikliniğine düzenli olarak takibe gelen ve HCV taşıyıcısı olan hastalarda HAV ve HBV serolojilerinin retrospektif olarak belirlenmesi, seronegatif olanların ise aşılanması amaçlanmıştır. Çalışmaya Nisan 2001-2006 tarihleri arasında infeksiyon hastalıkları polikliniğine başvuran ve hepatit C infeksiyonu saptanan hastaların tümü dahil edildi. Tümünde anti-HAV IgG, HBsAg, anti-HBs ve anti-HBcIgG serolojik belirleyicilerinin araştırılıp araştırılmadığı, dosya bilgileri incelenerek değerlendirildi. İncelenen serolojik işaretlerin tümü araştırılan 51 hastanın 35 (%68.6)'inde anti-HAV IgG, birinde (%1.9) HBsAg, yedisinde (%13.7) anti-HBs, 15'inde (%29.4) ise anti-HBs ve anti-HBc IgG pozitifliği saptandı. HAV için seronegatif olan 16 hastaya HAV aşısı; tüm HBV serolojik işaretleri negatif olan 28 hastaya da HBV aşısı yapılması sağlandı. Sonuç olarak; HCV ile infekte olan hastaların yaklaşık yarısı HBV için, %30'u ise HAV için seronegatif bulunmuştur. Bu hastalarda aşılama, olası HBV ve HAV infeksiyonlarının HCV infeksiyonunun seyrini olumsuz etkilemesini engelleyecektir.
