A case of diffuse tinea incognito associated with long-term systemic steroid use

Abstract

A 26-year-old male patient presented with a history of diffuse pruritus and rash on the body. Dermatological examination showed diffuse erythematous plaques with squamous surfaces on his hands, feet, inguinal area and intergluteal area. Subungual hyperkeratosis, onychodystrophy and onycho-dyschromia were observed on all hand and foot nails. The patient had been using intra-muscular betamethasone dipropionate weekly for 8 years. Native examination of the lesions on the hands, feet, inguinal area, hand and foot nails were evaluated positive, and Trichophyton rubrum grew in the fungal culture of the soles. The patient was diagnosed as diffuse tinea incognito associated with long-term systemic steroid use, and systemic and double topical antifungal treatment was started.
