Inflammatory fibroid polyp, mimicking a gastrointestinal stromal tumor

Abstract

Inflammatory fibroid polyp (Vanek's tumor) is a rare benign polypoid lesion of the gastrointestinal tract, measuring usually 1-3 cm. The histogenesis of the lesion is controversial. Histologically, it consists of spindled fibroblast-like cells intermingled with eosinophils. Whorls of fibroblasts or myofibroblasts surround thin-walled vessels in a concentric or onion-skin-like fashion. Differential diagnosis includes eosinophilic gastroenteritis and stromal tumors. We present a 49-year-old woman who was referred to our hospital with epigastric pain, weight loss and iron deficiency anemia. Abdominal ultrasonography and endoscopic ultrasonography revealed a 4 cm. submucosal lesion, suspicious for stromal tumor. thus an endoscopic biopsy was performed. Histologically the lesion was also suspicious for “Gastrointestinal stromal tumor” for presence of spindle cell proliferation, high Ki-67 index and CD-34 positivity in immunohistochemical survey, but the diagnosis was established as “Inflammatory fibroid polyp” in excision material. The case is presented to increase the awareness of the entity, for its rare occurrence, larger diameter and the potential risk of confusion with “Gastrointestinal stromal tumors”, in clinical, radiological and pathological aspects.
