Myomektomi sonrası karın duvarı endometriozisi

Endometriosis is characterized by the presence of histological normal endometrial tissue outside the uterine cavity. Ectopic endometrial tissue is most commonly seen in pelvic organs, ovaries, sacrouterine ligaments, the peritoneum, rectovaginal septum and cervix. Abdominal wall endometriosis is the ectopic location of the endometrial tissue in the abdominal wall and is usually preceded by surgery during which the uterine cavity is entered. Only 1% of endometriosis cases are located on the abdominal wall. Since it is rare, misdiagnoses are common and the diagnosis is usually delayed. The patient's cyclic symptoms and history of previous uterine surgery should suggest the correct diagnosis. Treatment is accomplished by total excision of the lesion. We present a case of abdominal wall endometriosis which occurred after myomectomy surgery, which is quite rare in the literature. The wide excision of the lesion was performed and the histopathological examination confirmed the presence of endometriosis.

Endometriosis histolojik olarak normal endometrial dokunun rahim içi boşluğun dışında bulunması ile karakterizedir. Ektopik endometrium dokusu en çok pelvik organlar, overler, Douglas boşluğu, sakrouterin ligamentler, pelvik periton, rektovaginal septum ve serviksi tutmaktadır. Karın duvarı endometriozisi ise genellikle histerotomi yapılan obstetrik veya jinekolojik operasyonlar sonrası endometriozisin karın duvarında yerleşmesidir. Tüm endometriozis olgularının ancak %1'i karın duvarı yerleşimidir. Nadir görülen bir hastalık olduğu için tanı genellikle gecikir ve tanıda yanılmalar yaygındır. Hastanın döngüsel şikayetleri ve geçirilmiş uterus cerrahisi öyküsü ile tanıya götürebilir. Tedavisinde total eksizyon yeterlidir. Burada literatürde oldukça nadir görülen myomektomi sonrası karın duvarında gelişmiş endometriozisli bir olguyu sunuyoruz. Lezyon geniş olarak eksize edildi ve histopatolojik inceleme endometriozis varlığını doğruladı.
