Akut glomerülonefritin nadir bir nedeni: Hepatit A virüs enfeksiyonu

Glomerulonephritis is the most common cause of both acute and chronic renal disease. The most common cause of acute glomerulonephritis is post-streptococcal, although viruses have also been implicated. Acute hepatitis A is one of the most common infectious diseases; it is usually a mild and self-limiting disease affecting the liver, but it sometimes leads to severe complications such as prolonged cholestasis and fulminant hepatitis. Although extrahepatic manifestations are not common, some cases have been reported associated with acute glomerulonephritis. Acute viral hepatitis-associated glomerular diseases are usually caused by hepatitis B and C viruses. Hepatitis A virus as a cause of acute glomerulonephritis has rarely been reported in the pediatric population. In this report, we describe a patient with serologically documented hepatitis A virus infection, who presented with nephritic syndrome.

Glomerülonefritler hem akut hem de kronik böbrek hastalıklarının en yaygın nedenidir. Akut glomerülonefritin en sık nedeni poststreptokokal olsa da virüsler de sorumlu tutulmuştur. Akut hepatit A, enfeksiyon hastalıklarının en yaygın olanlarından biri olup genellikle hafiftir ve karaciğerde kendi kendini sınırlar. Ancak bazen uzamış kolestaz, fulminan hepatit gibi ciddi komplikasyonlara yol açabilir. Karaciğer dışı bulguları nadir olmasına rağmen bazı vakalarda akut glomerülonefritle ilişkili olarak rapor edilmiştir. Akut viral hepatit ilişkili glomerüler hastalıklara genellikle hepatit B ve C virüsleri neden olmaktadır. Akut glomerülonefritin sebebi olarak hepatit A virüsü pediatrik popülasyonda çok nadiren bildirilmiştir. Bu yazıda nefritik sendromla başvuran, hepatit A virüs enfeksiyonu serolojik olarak belgelenmiş bir hastayı tanımladık.
