A STUDY ON HISTOLOGICAL PROGNOSTIC FACTORS AND PARTIAL SAMPLING METHODS İN RADICAL PROSTATECTOMY MATERIALS

Abstract

There is a lack of agreement upon the necessity of processing the radical prostatectomy materials (RPM) totally or partially, and if partial sampling is preferred which technique shall be used. İn our study, various partial sampling models were investigated for their sensitivity in determining the pathologic prognostic factors. We retrospectively reviewed 28 serially sectioned completely embedded RPMs in terms of total Gleason score (GS), vesicula seminalis invasion, capsular penetration, positive surgical margins, perineural invasion and high grade prostat prostatlc intraepithelial neoplasia. Five partial sampling models were designed considering the frequent localisation of prostate adenocarcinoma. The results were compared with the results of totaly embedded RPMs' using "Kappa consistency analysis method" for statistical analysis. İn Model 1; one section from 1/3 proximal, mid and distal portions of prostate, in Model 2; whole posterior lobe, in Model 3; whole posterior lobe plus one anterior lobe section from mid portion, in Model 4; every posterior lobe, in Model 5; every posterior lobe plus one anterior lobe section from mid portion is sampled. Apical and basal surgical margins and vesicula seminalis are included in each model. AH models appear to be consistent statistically. Sensitivity rates of sampling models for capsular penetration, positive surgical margins and perineural invasion varied between %69-%76 (K:0,70-0,78); %66-88 (K:0,73-0,91) and %94-100 (K:0,92-1), respectively. Ali models were able to detect GS>7 tumor except a GS 8 tumor "missed" in Model 4. We concluded that a logical partial sampling, considering the nature, prognostic parameters and frequent localisation of prostate tumors will provide similar results to the complete sampling of whole gland.
