Nimesulidin hepatotoksik etkisinin sıçanlarda araştırılması

Aim: Several cases of severe liver injury have been reported following the administration of nimesulide. The exact mechanism of hepatotoxic effect of nimesulide is yet to be clarified. The reactive metabolites that induce oxidative stress, has been suggested to be responsible for the liver damage as a result of the drug administration. However, it is well known that nimesulide has antioxidant activity as a consequence of its free radical scavenging activity. In this study, we aimed to investigate the hepatotoxic effect of nimesulide, the relationship of drug-induced liver damage and oxidative stress as well as antioxidant enzyme activities. Material and Method: Adult female rats were treated with nimesulide at doses of 2X2.5 (Group I) or 2X5.0 mg/kg/day (Group II) by intragastric gavage for 14 days. Control rats (Group K) received only 0.5 % carboxymethyl cellulose (CMC). Hepatotoxicity was monitored by histopathological evaluation of the livers and by measuring plasma malondialdehyde (MDA) level and serum biomarkers. Erythrocyte antioxidant enzymes activities were determined. Results: Nimesulide treatment caused significant increases in plasma MDA levels in both groups. Serum AST activities in Group I, and serum ALP activities in both nimesulide-treated groups, were significantly higher compared to Group K. In regard to antioxidant enzymes, a significant increase in the activity of catalase was observed in Group II. Histopathological examination of livers revealed mild to moderate damage, particularly in the rats treated with high dose of nimesulide. Conclusion: Our results indicate the hepatotoxicity potential of nimesulide and that oxidative stress might play a role for the development of liver injury in female rats.

Amaç: Nimesulid verilişinden sonra ciddi karaciğer hasarının görüldüğü çeşitli olgular rapor edilmiştir. Nimesulidin hepatotoksik etki mekanizması tam olarak aydınlatılmamıştır. Hasar oluşumundan oksidatif strese neden olan reaktif metabolitleri sorumlu tutulmaktadır. Ancak nimesulidin serbest radikalleri temizleyici özelliği nedeniyle antioksidan aktivite gösterdiği de bilinmektedir. Bu nedenle çalışmamızda, nimesulidin hepatotoksik etkisini ve hasar oluşumunda oksidatif stres ile antioksidan enzim aktivitelerinin ilişkisini araştırmayı amaçladık. Gereç ve Yöntem: Nimesulid, yetişkin dişi sıçanlara intragastrik gavajla, 2X2.5 (Grup I) ve 2X5.0 (Grup II) mg/kg/gün dozlarda, 14 gün süreyle uygulanmıştır. Kontrol grubunu oluşturan sıçanlara (Grup K) yalnızca % 0.5' lik karboksimetil selüloz (CMC) verilmiştir. Hepatotoksisite değerlendirmesi için karaciğer dokuları histopatolojik olarak incelenmiş, plazma malondialdehid (MDA) düzeyi ve serum göstergeleri ölçülmüştür. Eritrosit antioksidan enzim aktiviteleri tayin edilmiştir. Bulgular: Nimesulid uygulaması, her iki grupta da plazma MDA düzeylerinde anlamlı artışlara neden olmuştur. Serum AST aktiviteleri Grup I' de, serum ALP aktiviteleri ise nimesulid uygulanan her iki grupta, kontrol grubuna göre anlamlı düzeyde yüksek bulunmuştur. Antioksidan enzimlerden yalnızca katalaz enzim aktivitesi Grup II' de artmıştır. Karaciğerlerin histopatolojik incelemesi, yüksek doz nimesulid uygulanan sıçanlarda daha belirgin olmak üzere, hafiften orta dereceye değişen şiddette hasar oluştuğunu göstermektedir. Sonuıç: Bulgularımız, yetişkin dişi sıçanlarda nimesulidin hepatotoksik etki potansiyeline sahip olduğunu ve hasar oluşumunda oksidatif stresin rolü olabileceğini göstermektedir.
