Coronary artery calcium score is associated with red blood cell distribution width and mean platelet volume

Öz

Aim: In this paper, it
is aimed t
o investigate red blood cell
distribution width (RDW), mean platelet volume (MPV) and coronary artery
calcium scores (CACS) in patients with and without coronary artery disease
(CAD) detected by coronary computed tomography angiography (CCTA).



Materials and Methods: Clinical
records of 172 patients who underwent CCTA due to the presence of angina-like
symptoms were retrospectively investigated. Clinical characteristics, risk
factors for CAD, CACS and biochemical parameters including RDW and MPV were
recorded.

Results: Mean MPV (7.95 ±1.57 fL vs.
7.12±1.26 fL, p<0.001) and RDW (15.36±1.15% vs 14.37±2.01%; p<0.001)
levels were significantly higher in patients with CAD (n=85) than in those with
normal coronary arteries (n=87). CACS is positively corralated with age
(r=0.339, p<0.001), fasting glucose (r=0.223, p=0.003), C-reactive protein
(r=0.294, p<0.001), RDW (r=0.157, p=0.04) and MPV (r=0.221, p=0.004). On
multivariate logistic regression analysis, CACS [odds ratio (OR): 1.005; 95%
confidence interval (CI): 1.002-1.008; p=0.001] and MPV [OR: 1.410; 95% CI:
1.069-1.860; p=0.015] were shown to be one of the independent predictors for
CAD detected by CCTA.



Conclusion: RDW and
MPV were increased in patients with CAD detected by CCTA. These parameters were
possitively corralated with CACS which is a well-known indicator for CAD.
Because several factors may affect these parameters further prospective studies
are needed to confirm our results
.

Koroner arter kalsiyum skoru kırmızı kan hücresi dağılım genişliği ve ortalama trombosit hacmi ile ilişkilidir

Öz

Amaç: Bu çalışmada, koroner bilgisayarlı
tomografik anjiyografi (KBTA) ile saptanan koroner arter hastalığı (KAH) ve
koroner arter kalsiyum skorunun (KAKS) kırmızı kan hücresi dağılım genişliği
(RDW) ve ortalama trombosit hacmi (MPV) arasındaki ilişkinin değerlendirilmesi amaçlandı.

Gereç ve Yöntem: Anjina veya benzeri semptomlar nedeniyle 64 kesitli KBTA uygulanan 172
hastanın tıbbi kayıtları geriye dönük olarak incelendi. Hastaların klinik
özellikleri, risk faktörleri, KAKS’ı da içeren KBTA sonuçları, RDW, MPV
değerlerini de içeren biyokimyasal ve hematolojik parametreleri kaydedildi.

Bulgular: Çalışma popülasyonu KBTA sonuçlarına göre KAH (n=85) ve normal koroner
arter (NKA) (n=87) olmak üzere iki gruba ayrıldı. Ortalama RDW (%15,36±1,15’e
karşı %14,37±2,01; p<0,001) ve MPV (7,95 ± 1,57 fL’e karşı 7,12±1,26 fL,
p<0,001) seviyeleri KAH grubunda NKA grubuna göre anlamlı olarak yüksek
saptandı. KAKS ile yaş (r=0,339, p<0,001), açlık kan şekeri (r=0,223, p=0,003),
C-reaktif protein (r=0,294, p<0,001), RDW (r=0,157, p=0,04) ve MPV (r=0,221,
p=0,004) arasında pozitif korelasyon saptandı. Yapılan çok değişkenli lojistik
regresyon analizinde KAKS [odds oranı (OR): 1,005; %95 güven aralığı (CI): 1,002-1,008;
p=0,001] ve MPV [OR: 1,410; %95 CI: 1,069-1.860; p=0.015] KBTA ile saptanan
KAH’ın bağımsız risk faktörleri arasında saptandı.







Sonuç: RDW ve MPV KAH grubunda anlamlı olarak daha yüksek saptanmıştır. Koroner
aterosklerozun iyi bir göstergesi olan KAKS ile bu parametreler arasında
pozitif bir korelasyon gösterilmiştir. MPV ve RDW düzeylerinin çok çeşitli
faktörlerden etkilenebileceği de düşünülürse bu konuda geniş kapsamlı ileri prospektif
çalışmalara ihtiyaç duyulmaktadır
