Madde kullanım bozukluğu olan bireylerde hepatit ve HIV seroprevalansı

Abstract

Amaç: Madde kullanım bozukluğu (MKB), güvenli olmayan enjeksiyon uygulamaları, riskli davranışlar ve olumsuz yaşam koşulları nedeniyle özellikle viral hepatitler ve HIV gibi kan yoluyla bulaşan enfeksiyonlar açısından önemli bir risk faktörü olarak kabul edilmektedir. Bu çalışmada MKB tanılı bireylerde hepatit A virüsü (HAV), hepatit B virüsü (HBV), hepatit C virüsü (HCV) ve HIV seroprevalansının belirlenmesi ve bu popülasyondaki tarama uygulamalarının değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Bu retrospektif çalışmada, 1 Haziran 1998–1 Temmuz 2025 tarihleri arasında üçüncü basamak bir sağlık kurumunda bulunan psikiyatri kliniğinde MKB tanısıyla ayaktan takip edilen hastaların tıbbi kayıtları incelenmiştir. Hastaların demografik özellikleri, madde kullanım özellikleri ile HAV, HBV, HCV ve HIV serolojik sonuçları hastane kayıtlarından elde edilmiştir. Seropozitiflik oranları hesaplanmış ve tarama uygulamaları değerlendirilmiştir. Bulgular: Çalışma döneminde toplam 8.145 hastaya ait kayıt incelenmiş, bunlardan en az bir serolojik test sonucu bulunan 2.478 hasta (%30,4) analize dahil edilmiştir. Hastaların medyan yaşı 33 yıl (ÇAA: 25–45) olup, %85,6’sı erkektir. Test edilen hastalar arasında anti-HAV IgG pozitifliği %63,6 olarak saptanmıştır. HBsAg pozitifliği %2,3, anti-HCV pozitifliği %1,9 ve HIV seropozitifliği %0,85 olarak bulunmuştur. Bununla birlikte hastaların önemli bir kısmında viral hepatitlere yönelik testlerin eksik olduğu ve kapsamlı taramanın yalnızca sınırlı sayıda hastada yapıldığı görülmüştür. Sonuç: MKB tanılı bireyler viral hepatitler ve HIV enfeksiyonu açısından risk altında olmaya devam etmektedir. Ancak bu popülasyonda tarama uygulamalarının yeterli düzeyde olmadığı görülmektedir. Bağımlılık poliklinikleri tarama, erken tanı ve tedaviye yönlendirme açısından önemli bir fırsat sunmaktadır. Standart tarama stratejilerinin uygulanması, bu enfeksiyonların saptanma oranlarını ve yönetimini iyileştirebilir.

Seroprevalence of hepatitis and HIV among individuals with substance use disorder

Abstract

Aim: Substance use disorder (SUD) is associated with a disproportionately increased risk of blood-borne infections, particularly viral hepatitis and HIV, due to unsafe injection practices, high-risk behaviors, and precarious living conditions. This study aimed to determine the seroprevalence of hepatitis A virus (HAV), hepatitis B virus (HBV), hepatitis C virus (HCV), and HIV among individuals with SUD and to evaluate screening practices in this population. Materials and Methods: In this retrospective study, medical records of patients diagnosed with SUD and followed in a tertiary care psychiatry clinic between June 1, 1998 and July 1, 2025 were reviewed Demographic characteristics, substance use patterns, and serological results for HAV, HBV, HCV, and HIV were obtained from hospital records. Seropositivity rates were calculated and screening practices were evaluated. Results: A total of 8,145 patient records were screened, and 2,478 patients (30.4%) who had at least one serological test performed were included in the analysis. The median age was 33 years (IQR: 25–45), and 85.6% were male. Anti-HAV IgG positivity was detected in 63.6% of tested patients. HBsAg positivity was found in 2.3%, anti-HCV positivity in 1.9%, and HIV seropositivity in 0.85%. A considerable proportion of patients had incomplete viral hepatitis testing, and comprehensive screening was performed in only minority of patients. Conclusions: Although individuals with SUD remain at risk for viral hepatitis and HIV infections, screening practices in this population appear to be insufficient. Addiction outpatient clinics represent an opportunity for screening, early diagnosis, and linkage to care. Implementing standardized screening strategies may improve detection and management of these infections.
