TW0 YEARS EXPERIENCE OF HOLTER MONITORING IN CHILDREN

Abstract

Cardiac arrhythmia in pediatric patients appears as a significant problem in recent years, with the increase in the number of post-operative cases. The resting electrocardiographic findings give information only about a short period of cardiac rhythm, and accordingly most of the arrhythmias may be skipped. In this retrospective study, we analysed the prognostic significance and effectiveness of 24-Hour Holter monitoring recordings in pediatric cardiac patients. In this study, 490 Holter monitoring recordings obtained from 367 cases in Ege University Medical Faculty Pediatric Cardiology Department, from February 1999 to March 2001 were analysed retrospectively. The age of the patients were betvveen 6 days and 28 years and their average age was 11±5 years. The Holter monitoring recordings of the patients were divided into three groups as; asymptomatic dysrhythmic group, symptomatic non-dysrhythmic group and both symptomatic and dysrhythmic group. While 54% of the cases vvere normal, dysrhythmia of a wide spectrum was observed in 46% of the cases. The distribution of these findings vvere ventricuiar dysrhythmia 25%, supraventricular dysrhythmia 10%, supraventricular and ventricuiar dysrhythmia together 5%, complete A-V block 4%, and Long QT syndrome was 2%. İn symptomatic cases, it was difficult to determine cardiac arrhythmia during Holter Monitoring since no symptom was observed. Nevertheless, we consider that the prognostic significance of Holter monitorization is high in the cases who are suspicious about dysrhythmia.
