IS THERE ANY CHANGE IN THE LOCATION OF GASTRIC ADENOCARCINOMAS ?

Abstract

Although it is known for developed counthes, that there has been a recent increase in the incidence of adenocarcinoma of the esophagus and cardia, along with a decrease in distal gastric cancers within the last 30 years. Little is known regarding the prevalence of these diseases in developing counthes. The changing prevalence of localizations of gastric cancer is particularly important in these countries since their incidence of Helicobacter pylori (HP) is now markedly high and health çare accessibility and guality of life are not uniformly high. The aim of the study was to evaluate the changes in the incidence of gastric adenocarcinomas in Aegean Region. Data were retrospectively collected from of Ege University Faculty of Medicine, Section of Gastroenterology from January 1990 to December 2000, yielding 499 cases of adenocarcinoma of the stomach and distal esophagus. Tumors localized to the body, the antrum, and pyloric channel were considered distal cancers. H. pylori was detected either with histology or rapid urease test. Distal gastric adenocarcinomas were significantly more common than proximal gastric adenocarcinomas. The ratio of distal vs proximal gastric adenocarcinomas has not being changed within last 11 years. This study showed that Aegean Region, as a part of a developing countrv with high HP incidence, reoresents a different profile than the developed countries, as manifested by the dominance of distal adenocarcinomas. Contrary to the state of the developed countries, the ratio of distal vs proximal gastric adenocarcinomas did not change within last 11 years.
