THE EFFECTS OF LIDOCAINE AND DILTIAZEM ON HAEMODYNAMIC CHANGES DURING TRACHEAL EXTUBATION

Abstract

Increased sympathetic response to surgical stimulation in craniotomy cases, comes out at the end of the operation with discontinuation of anesthetics. Depressing the haemodynamic response on these cases is important for the postoperative prognosis of the patients. In this study, the haemodynamic effects of diltiazem and lidocaine given intravenously just before extubation in patients undergoing craniotomy, is investigated. With approval by the Local Ethics Committee of the University Hospital of Ege and after obtaining informed consent, 30 patients ASA I - II, between the ages 18-60 were randomly assigned to two groups. After standardised induction of anesthesla, 50-50% N2O - O2 and sevoflurane 2-2.5 % were used for the malntenance of anesthesia. Intraoperative haemodynamic changes were recorded. In Group D patients received dlltiazem 0.2 mg/kg, in Group L patients received lidocaine 0.1 mg/ kg intravenously 2 minutes before extubation. Heart rate (HR), systolic arterial pressure (SAP), mean arterial pressure (MAP) were recorded just before extubation, after extubation, 1., 2., 5. and 10. min after extubation. RPP (rate pressure product) were calculated from (RPP = HR x SAP) formulas. Assesment for the quality of tracheal extubation was made with 5 points scale when extubation performed.
