DO THE ELDERLY HAVE ADEOUATE INFORMATION ABOUT THE DRUGS THEY USE ?: RESULTS OF A STUDY CONDUCTED İN A PRIMARY HEALTH ÇARE İN TRABZON

By the development of pharmacotherapy, polipharmacy and drug interactions and side affects increases, especially in elderly people. This study was carried out to evaluate the compliance and the level of knowledge about drug using of the elderly people. Four hundred (77.7%) elder inhabitants (total 515) of Trabzon Bahçecik Primary Çare Region, seen in home visits wem enrolled the study. Data was analysed with student t, chi-square tests and enter-method logistic regression. It was found that 311(77.8%) of ali had a chronic syndrome and 283 (91 %9) of them use drugs continuousiy. The most common syndromes were hypertension, musculoskeletal diseases, coronary heart disease and diabetes mellitus respectively. 25.4% of ali use just one drug, 97.2% of the drugs were prescribed by a doctor. 52.3% of the people can supply their drugs by themselves. 100 people stopped their medication beyond their doctor's decision, because of any other unrelated acute problem in the last year. Education and read of drug prospectus have positive effect, and increasing the number of daily drug has negative effect on drug knowledge level of elderly (OR=2.91, 95% CI:1.31-6.48; OR=0.61, 95% CI.0.50-0.74). As a result, elderly people are have to been accepted as one of the preceding groups in planning of the health service. İt should not been forget that the medical treatment of geriatric diseases could be become conscious of the elderly about their diseases and drug, and the getting a team with the family.

Tedavide sağlanan gelişmeler, genellikle birden çok hastalığın gözlendiği yaşlılarda kullanılan ilaç çeşitliliğini, bu nedenle ilaç etkileşimleri ve yan etki görülme olasılığını arttırmaktadır. Bu çalışma yaşlıların ilaç kullanma bilgi düzeylerini ve ilaç uyumunu incelemek amacıyla yapılmıştır. Veriler student t ve ki-kare testleri ile lojistik regresyon analizi ile analiz edilmiştir. Yaşlıların 311'inin (%77.8) kronik hastalıklarının olduğu, bunların da 283'ünün (%91.0) sürekli ilaç kullandığı saptanmıştır. Bu hastalıklar sırasıyla hipertansiyon, romatizmal hastalıklar, aterosklerotik kalp hastalıkları ve diabetes mellitustur. Yaşlıların %25.4'ünün günde tek çeşit ilaç kullandığı, kullandıkları ilaçların %97.2'sinin doktor tarafından önerildiği ve %52.3'ünün ilacı kendisinin temin ettiği bulunmuştur. Son biryıl içinde akut bir rahatsızlığı olan 100 yaşlının 16'sının (%16.0) bu dönemde ilaçlarını kestikleri saptanmıştır. Yaşlıların ilaçlarını bilme durumları üzerine örgün eğitim alma ile ilaç kullanım bilgisi kağıdını okumanın olumlu, günde alınan ilaç sayısının artmasının ise olumsuz etkisinin olduğu bulunmuştur (OR=2.91, 95% CI:1.31-6.48; OR=0.61, 95% CI:0.50-0.74). Sağlık hizmetleri planlanırken, yaşlıların öncelikli gruplardan biri olduğu bilinmeli ve tedavi edici geriatrik hizmetlerin yaşlıların hastalıkları ve ilaçları hakkındaki bilinçlendirilmeleri ve aileyi de içeren ekip anlayışı ile sağlanabileceği unutulmamalıdır.
