A patient with abdominal, pelvic and hepatic actinomyces abscesses who had convulsions due to penicillin treatment: Case report and review of the literature

Abstract

Actinomyceses which do not have the painting characteristic of an acid alkali resistant-bacteria are facultative or obligatory anaerobic, nonsporulating gram-positive bacillus. We intended to evaluate the diagnosis and the treatment of an Actinomycosis case with multiple hepatic, abdominal and pelvic abscesses that had fistula to colon and subcutaneous location and who had convulsions due to penicillin therapy with the accompaniment of literature at this presentation. The 45 year old male patient presented with right upper quadrant pain which had started a month previous and increased progressively, accompanied by a weight loss about 10 kg, fever and watery diarrhea. Multiple hepatic and intraabdominal abscesses were diagnosed. Tissue samples taken from the hepatic abscess were determined to be an Actinomycosis infection. Twentyfour million units/day of crystallized penicillin treatment was started. Due to epileptic attacks during treament, the penicilin dosage was decreased. In patients with widespread abscesses in the liver, abdomen and pelvis, the occurence of Actinomyces must be kept in mind. Upon observing sulfur granules and dense hypheas with cytological illustration in patients with appropriate settlement, Actinomycosis can be diagnosed easily. It must be kept in mind that during the treatment convulsive attacks due to high doses of penicilin for a prolonged period may be observed.
