Laparoscopic Burch colposuspension for the treatment of stress urinary incontinence: 5-year follow-up results

Abstract

Objective: To assess the outcomes of transperitoneal laparoscopic burch colposuspension in patients with stress urinary incontinence. Materials and methods: 24 patients with stress urinary incontinence who underwent transperitoneal laparoscopic Burch colposuspension between 2002-2007 at the Ege University Medical Faculty, Department of Obstetrics and Gynecology were evaluated retrospectively. Two laparocopic hysterectomies, seven laparoscopic hysterectomy and bilateral salpingo-oophorectomies and one tubal ligation were performed on these patients, additionally. In the pre-operative period, all patients were evaluated with a medical history, urinary analysis, cotton swab test, and urodynamic test. In the postoperative period, each patient was evaluated with a simple cystometry and cough stress test at 6 weeks, 6 months and every 6 months after operation. The mean BMI was 26±2.3 kg/m2, age 46.8±4.5 years, parity 2.2±1.2 and duration of urinary incontinence was 3.2±1.4 years. Ten patients (41%) had a history of previous pelvic surgery and fourteen patients (59%) were in the postmenopausal period. Results: The mean length of hospital stay was 2.1±1.3 days, blood loss was 70±35 ml, duration of operation was 65±14 minutes, and duration of urinary catheterization was 1.55±1.54 days. Twenty-one patients (87.5%) were continent after their operations during 60 months follow-up. One urinary retention (catheterization, seven days), one post-colposuspension syndrome with difficult urination (urethrolysis via laparotomy), three de-novo detrusor instability (treated with oxybutynin), and four urinary catheterizations for more than 24-hours were detected in the postoperative period. Conclusion: The transperitoneal laparoscopic Burch colposuspension was an effective method for urinary stress incontinence because it reduced the length of hospital stay, resulted in minimal blood loss, and had an acceptable complication rate.
