Abstract
Inflammatory fibroid polyp (Vanek's tumor) is a rare benign polypoid lesion of the gastrointestinal tract, measuring usually 1-3 cm. The histogenesis of the lesion is controversial. Histologically, it consists of spindled fibroblast-like cells intermingled with eosinophils. Whorls of fibroblasts or myofibroblasts surround thin-walled vessels in a concentric or onion-skin-like fashion. Differential diagnosis includes eosinophilic gastroenteritis and stromal tumors. We present a 49-year-old woman who was referred to our hospital with epigastric pain, weight loss and iron deficiency anemia. Abdominal ultrasonography and endoscopic ultrasonography revealed a 4 cm. submucosal lesion, suspicious for stromal tumor. thus an endoscopic biopsy was performed. Histologically the lesion was also suspicious for “Gastrointestinal stromal tumor” for presence of spindle cell proliferation, high Ki-67 index and CD-34 positivity in immunohistochemical survey, but the diagnosis was established as “Inflammatory fibroid polyp” in excision material. The case is presented to increase the awareness of the entity, for its rare occurrence, larger diameter and the potential risk of confusion with “Gastrointestinal stromal tumors”, in clinical, radiological and pathological aspects.
Abstract
İnflamatuvar fibroid polip (Vanek tümörü), gastrointestinal traktusun nadir görülen, çapı genellikle 1-3 cm arasında değişen, histogenezi tartışmalı, benign polipoid bir lezyonudur. Histolojik olarak, damarlar etrafında konsantrik dizilim göstermiş, fibroblast benzeri prolifere iğsi hücreler ve eozinofillerden zengin yangısal bir infiltrasyonla karakterlidir. Ayırıcı tanısında öncelikle, eozinofilik gastroenterit ve stromal tümörler akla gelir. Epigastrik ağrı, kilo kaybı ve demir eksikliği anemisi nedeniyle başvuran 49 yaşındaki bir kadın hasta sunulmuştur. Olgunun batın ultrasonografisi ve endosonografisinde 4 cm. çapında, stromal bir tümör kuşkusu uyandıran submukozal bir kitle tespit edilmiş ve endoskopik biyopsi yapılmıştır. Biyopside, iğsi hücre proliferasyonu, CD-34 pozitifliği ve yüksek Ki-67 proliferasyon indeksi nedeniyle öncelikle “Gastrointestinal stromal tümör” kuşkusu duyulmuş, takip eden eksizyon materyalinde “İnflamatuvar fibroid polip” tanısı konmuştur. Olgu, nadir görülmesi, alışılagelenden büyük çaplı olması, klinik, radyolojik ve histolojik olarak Gastrointestinal stromal tümörlerle karışabilme riski nedeniyle bu konudaki farkındalığı artırmak amacıyla sunulmuştur.
