A case of renal cell cancer presenting with tumor thrombus extending into the vena cava inferior and right atrium

Abstract

With the increasing use of imaging methods over time, the number of solid renal masses detected incidentally has increased. Characterization of incidental solid renal masses is very important for accurate diagnosis, treatment, and follow-up. Renal cell cancer (RCC), which constitutes approximately 80-90% of primary renal neoplasms, is known as the most common solid renal neoplasia and is among the frequently diagnosed adult cancers. Renal cell cancer tends to extend through the tumor thrombus to the renal veins, the main renal vein, the inferior vena cava (IVC) and potentially the right atrium. Only 10% of tumor thrombi that extend to the IVC reach the right atrium. Malignant thrombus extension accompanying RCC is at the center of the planned surgical treatment, therefore it is very important to detect malignant thrombus with imaging methods and evaluate its extension.

Vena kava inferior ve sağ atriyuma uzanan tümör trombüsü ile prezente olan renal hücreli kanser olgusu

Abstract

Zaman içerisinde görüntüleme yöntemlerinin kullanım sıklığının artması ile rastlantısal olarak saptanan solid renal kitlelerin sayısı artmıştır. Doğru tanı, tedavi ve takip süreci için rastlantısal solid renal kitlelerin karakterizasyonları oldukça önemlidir (1). Primer renal neoplazilerin yaklaşık %80-90’ını oluşturan renal hücreli kanser, en yaygın solid renal neoplazi olarak bilinmekte ve sık teşhis edilen yetişkin kanserlerinin arasında yer almaktadır. Renal hücreli kanser, tümör trombüsü yolu ile renal venlere, ana renal vene, vena kava inferior’a ve potansiyel olarak sağ atriyuma uzanım gösterme eğilimindedir. Vena kava inferior’a uzanım gösteren tümör trombüslerinin ise sadece %10’u sağ atriuma ulaşmaktadır (2). Renal hücreli kansere eşlik eden malign trombüs uzanımı, planlanan cerrahi tedavinin merkezinde yer almakta, bu nedenle de görüntüleme yöntemleri ile malign trombüsün saptanması ve uzanımının değerlendirilmesi oldukça önem arz etmektedir.
