Percutaneous nephrolithotomy: Our experience in the first 110 renal units

Abstract

Aim: To present our experience of percutaneous nephrolithotomy (PNL) in the first 110 renal units. Material and Methods: One hundred and five patients who had renal stones were enrolled in this study. Five patients had bilateral renal stones. Percutaneous renal access was done under C-armed flouroscopy with patients in the prone position. The tract was dilated with an Amplatz dilatator set and the stones were fragmented with a 26 Fr rigid nephroscope and pneumatic lithotriptor. Results: One hundred and ten procedures were performed on the patients. The mean age was 42.6 years (range 16-72), stone burden was 5.2 (range 2-16) cm2, operative time was 115.3 (range 60-240) minutes and hospitalization time was 3.8 ( range 3-8 ) days. Total success rate was 78.1 % (86/110). The procedures were terminated in 3 (2.7 %) procedures due to bleeding in two patients and colonic perforation in one. The blood transfusion rate was 11.8 %. Fever (>38.5°C), hydropneumothorax, pelvic laceration, retroperitoneal migration of stones and prolonged urinary drenaj were seen in 5 (4.7 %), 1 (0.9 %), 5 (4.7 %), 1 (0.9%), 2 (1.8%) procedures, respectively. Auxiliary procedures were needed in 8 (7.6 %) patients (Second look PNL in 3 patients, URSL in 1, DJ stent in 3 and torax tube in 1) Conclusion: PNL can be considered as a an effective and safe treatment modality for the management of renal stones with an acceptable morbidity and success rate.

Öz

Amaç: İlk 110 renal ünitedeki perkütan nefrolitotomi (PNL) deneyimimizi sunmak. Gereç ve Yöntem: Böbrek taşlı 105 hasta çalışmaya alındı. Olguların beşinde bilateral böbrek taşı vardı. Perkütan renal giriş, C-kollu floroskopi eşliğinde, prone pozisyonunda yapıldı. Trakt amplatz dilatatör seti ile dilate edildi. Taşlar 26 F rijid nefroskop ve pnömatik taş kırıcı ile kırıldı. Bulgular: Hastalara 110 işlem uygulandı. Ortalama yaş 42.6 yıl (16-72), taş boyutu 5.2 (2-18) cm2, operasyon süresi 115.3 (60-240) dakika ve hastanede yatış süresi 3.8 (3-8) gün idi. Genel başarı oranı 78.1% idi. İki olguda kanama, bir olguda ise kolon perforasyonu nedeniyle operasyon sonlandırıldı. Kan transfüzyon oranı % 11.8 idi. Ateş (>38.5°C), hidropnömotoraks, pelvik laserasyon, retroperitona taş migrasyonu, uzamış idrar drenajı oranları sırasıyla; 5 (%4.7), 1 (%0.9), 5 (%4.7), 1 ( %0.9), 2 (%1.8) idi. Olguların üçünde ikinci kez (second look) PNL, birinde üreterorenoskopi eşliğinde taş kırma (URSL), üçünde DJ stent, birinde toraks tüpü olmak üzere toplam sekizine (% 7.6) ek girişim uygulandı. Sonuç: Böbrek taşlarının tedavisinde PNL kabul edilebilir morbidite ve başarı oranına sahip, etkili ve güvenilir bir tedavi yaklaşımıdır.
