DISTRIBUTION OF EBV-LMP 1 EXPRESSION ACCORDING TO HISTOPATHOLOGİCAL SUBTYPES OF CLASSICAL HODGKIN'S LYMPHOMAS
Abstract
Epstein –Barr virus plays role in the pathogenesis of classical Hodgkin's lymphomas. Epstein Barr virus latent membrane protein-1 ( EBV-LMP1 ) is usually expressed in Hodgkin and Reed Sternberg ( HRS ) cells. In this study, the immunohistochemical expression of EBV-LMP1 in classical Hodgkin's lympoma cases and its distribution according to histopathological subtypes were investigated. In Ankara Oncology Hospital, Department of Pathology, 37 cases; diagnosed as Hodgkin's lymphoma between 1998-2004 with adequate tissue samples to study, were included in this study group. Immunohistochemically, staining with anti EBV-LMP1 antibody was applied. In 19 of 37 classical Hodgkin's lymphoma cases (51,35%) EBV-LMP1 expression was observed. In 11 of 13 mixed cellularity subtype (84,6%), three of 14 nodular sclerosis subtype (21,4%), five of six lymphocyte depleted subtype (83,3%) EBV-LMP1 expression was seen in HRS cells. No expression was observed in all of four lymphocyte rich subtype. In classical Hodgkin's lymphoma cases, the expression of EBV-LMP1 supports that EBV infection plays a role in pathogenesis. Among the histopathological subtypes highest expression ratio was observed in mixed cellularity type, lowest ratio was observed in lymphocyte rich type. Other agents must be researched in the pathogenesis of the cases which are not associated with EBV infection.

Epstein-Barr virüs, klasik Hodgkin lenfomaların patogenezinde yer almaktadır. Epstein-Barr virüs latent membran protein-1 (EBV-LMP1) Hodgkin ve Reed Sternberg (HRS) hücrelerinde sıklıkla eksprese edilmektedir. Bu çalışmada klasik Hodgkin lenfoma olgularında immünhistokimyasal olarak EBV-LMP1 ekspresyonu ve histopatolojik subtiplere göre dağılımı araştırılmıştır. Ankara Onkoloji Hastanesi patoloji bölümünde 1998-2004 yılları arasında klasik Hodgkin lenfoma tanısı alan ve çalışma için yeterli doku örnekleri mevcut olan toplam 37 olgu çalışma grubunu oluşturmaktadır. İmmünhistokimyasal olarak anti-EBV-LMP1 antikoru ile boyama yapılmıştır. Otuzyedi klasik Hodgkin lenfoma olgusunun 19'unda (%51,35) EBV-LMP1 ekspresyonu izlenmiştir. Onüç mikst selüler subtipin 11'inde (%84,6), 14 nodüler sklerozan subtipin üçünde (%21,4), altı lenfositten yoksun subtipin beşinde (%83,3) EBV-LMP1 ile HRS hücrelerinde boyanma olmuştur. Dört lenfositten zengin subtipin hiçbirinde boyanma izlenmemiştir. EBV-LMP1'in klasik Hodgkin lenfoma olgularında ekspresyonu, EBV enfeksiyonunun patogenezde rolü olduğunu desteklemektedir. Histopatolojik subtiplerden en yüksek oranda ekspresyon mikst selüler tipte, en düşük oranda ekspresyon ise lenfositten zengin tipte izlenmiştir. EBV enfeksiyonu ile ilişkili olmayan olgularda patogenezde başka ajanların araştırılması gerekmektedir.
